Care Options

A continuum of support designed around each person’s needs.

Structured clinical support across multiple levels of care, including licensed residential programs, transitional living, supportive apartments, case management, and individualized in-home support.

Each option offers a different balance of structure, independence, daily support, clinical case management, and community connection.

The goal is not to fit someone into a program simply because a space is available. The goal is to understand what level of support may help that person move forward safely, thoughtfully, and successfully.

Support that can meet people where they are.

Some individuals benefit from the consistency of a residential setting. Others may be ready for transitional living, apartment-based support, or individualized services in their own home.

Wild Acre may serve as a step-down from a higher level of care, or as a more supportive setting when outpatient treatment alone is not enough.

Because needs can change over time, our continuum allows support to evolve as individuals build stability, strengthen skills, and work toward greater independence.

You do not have to choose the right option alone.

Choosing the right level of care can feel overwhelming. Families, referral sources, and individuals often come to Wild Acre unsure which program is the best fit.

That is where our admissions process begins.

Every prospective resident participates in a comprehensive admissions assessment with our Director of Admissions and Clinical Services. This includes conversations with the individual, family members when appropriate, and referring providers.

Placement recommendations consider the whole person, including:

Current level of functioning
Psychiatric stability
Daily living skills
Executive functioning needs
Medication support needs
Treatment history
Outpatient providers and supports
Family involvement
Community readiness
Motivation for treatment
Goals for greater independence

If Wild Acre is an appropriate fit, our team recommends the program or level of support that best matches the individual’s clinical needs, support requirements, personality, goals, and readiness for independence.

Each care option offers a different balance of structure, independence, clinical case management, and daily support. You do not need to choose the right option before reaching out, but these pathways can help you understand the general differences.

Residential Support

For adults who benefit from 24-hour staff availability and a stable, home-like setting.

May be appropriate for individuals who need consistent structure, clinical case management, daily routine, and ongoing support while working toward stability and quality of life.

Transitional Living

For adults preparing for a greater level of independence.

Short-term residential support for individuals working to stabilize, rebuild skills, strengthen routines, and prepare for a less restrictive living environment.

Supportive Apartments

For adults ready for more independence with continued support.

May be appropriate for individuals who can live more independently while continuing to benefit from clinical case management, peer connection, and support as needed.

Home Care Services

For adults living in their own homes who need practical daily support.

Individualized support for adults who do not need residential placement but would benefit from help maintaining routines, attending appointments, engaging in the community, and working toward personal goals.

Support can change as needs change.

The right starting point is important, but it is not always the final destination.

As residents make progress, or as our team learns more about what type of environment best supports their goals, Wild Acre may recommend a transition to another program or level of care within the continuum.

This flexibility helps individuals receive the level of structure, support, and independence that best matches their needs throughout their treatment journey.

You do not need to know which program or level of care is right before reaching out.
