Home Care Services are customized around each person’s needs, goals, routines, outpatient care, and community life. Support can help maintain existing stability or carry skills and routines from residential, transitional, hospital-based, or apartment-based care into a more independent setting—without disrupting established relationships or community connections.

Who Home Care Services may be right for

Home Care Services may be appropriate for adults who are able to live outside of a residential program but would benefit from individualized support in daily life.

This level of care may support individuals who need help with routines, medication practices, appointments, symptom management, transportation, vocational goals, community engagement, or maintaining progress after a higher level of care.

Home Care Services may be helpful for adults who are:

Living independently but needing additional structure and accountability
Transitioning from a residential, transitional, hospital-based, or apartment-based setting
Working to maintain routines, appointments, and treatment relationships
Building confidence with community participation
Managing symptoms while living at home
Needing support with medication routines, organization, or monitoring
Reconnecting with vocational, educational, volunteer, or other meaningful goals
Benefiting from individualized support rather than a group living environment

Home Care Services are available to adults living in the community and are not limited to individuals who have previously participated in a Wild Acre program.

Services are provided on a private-pay, fee-for-service basis and are tailored to each person’s goals and level of support needed.

What Support Can Look Like

Flexible support shaped around individual needs.

Home Care Services are highly individualized. Support can be adjusted over time as needs change, allowing for greater support during more challenging periods and more independence as skills, routines, and confidence develop.

The frequency of visits depends on each person’s needs. Some clients receive support daily, several times per week, weekly, or on a more intermittent basis. The minimum service visit is one hour, allowing flexibility while still providing meaningful, individualized support.

Support may include:

Schedule and calendar management
Appointment scheduling and organization
Medication organization, ordering, refill coordination, and pharmacy pick-up assistance
Medication monitoring, when needed
Nursing care, when clinically appropriate
Transportation to medical, clinical, and other essential appointments
Vocational, educational, or volunteer support
Symptom management and emotional support
Coping skills coaching and reinforcement
Support engaging in the community and building confidence in social situations
Collaboration with outpatient providers and treatment teams
Budgeting, grocery shopping, and household organization
Coaching around routines, executive functioning, and daily responsibilities
Assistance identifying and building meaningful weekly structure
Support transitioning from a higher level of care to greater independence

When clinically appropriate, nursing support may include medication injections, health monitoring, or accompaniment to medical appointments.

Home Care support is designed to help individuals apply skills in the places where they use them every day: at home, during appointments, in the community, and within their regular routines.

Support might include helping someone organize the week ahead, prepare for a medical or clinical appointment, follow through with medication routines, practice coping strategies, manage daily responsibilities, or build confidence navigating the community.

Because support takes place in the person’s own environment, staff can help identify practical barriers, reinforce strengths, and create strategies that are realistic and sustainable.

The focus is helping individuals build confidence, consistency, and greater independence while supporting the responsibilities and routines that contribute to long-term stability.

Transitions can be difficult, especially when someone is moving from a more structured level of care into a more independent living situation.

Home Care Services can help bridge that gap by providing individualized support as people build confidence maintaining their independence.

Home Care is commonly used as a step-down service for individuals transitioning from Wild Acre’s residential and Supportive Apartment programs, allowing for continuity of care with a familiar team. It may also be appropriate for individuals transitioning from other residential, hospital, or structured treatment settings who would benefit from ongoing support in the community.

Wild Acre can provide support as individuals adjust to new routines, reconnect with outpatient providers, manage appointments, continue medication practices, build meaningful weekly structure, and remain engaged in their communities.

Transition planning may begin before discharge from a higher level of care. Whenever possible, Wild Acre collaborates with the individual, family, and treatment providers to develop a plan that promotes continuity, minimizes disruption, and supports a successful transition into the community.

For families, this can offer reassurance that their loved one is not navigating the transition alone. For the individual, it provides continued accountability, familiar support, and guidance while practicing greater independence over time.
