Coordinated care with experienced mental health clinicians.
Belmont House provides individualized clinical case management from experienced mental health clinicians.
Each resident works closely with a designated clinical case manager who supports treatment planning, goal setting, daily structure, outpatient coordination, and communication with family members and other treatment team members when appropriate.
Belmont House also provides medication support through staff certified by the Massachusetts Medication Administration Program. Staff assist with medication ordering, secure storage, administration, communication with prescribing providers, and skill-building toward medication independence when clinically appropriate.
Belmont House does not provide on-site psychiatric treatment. Residents continue working with their outpatient psychiatrist for medication management and psychiatric care. For residents who need help establishing outpatient psychiatric care, Wild Acre can assist with referrals and connections to trusted community providers.
