Treatment-Team Collaboration

Wild Acre works with outpatient providers, hospitals, psychiatrists, therapists, case managers, educational consultants, and other referral professionals to identify the appropriate level of support.

Across residential support, transitional living, supportive apartments, and home care services, clinical case management and coordinated care help maintain treatment continuity while creating an individualized plan around each person’s needs and goals.

Making a Referral

Referrals may be made by an individual, family member, clinician, psychiatrist, therapist, case manager, hospital, educational consultant, or other referral source.

The process begins with a confidential initial screening conversation with the Director of Admissions and Clinical Services to learn more about the individual’s current needs, treatment history, goals, and potential fit.

If Wild Acre appears to be a potential fit, the Director of Admissions may request relevant clinical information to further assess appropriateness for admission.

Helpful referral materials may include:

Recent clinical summaries
Discharge paperwork
Psychiatric or diagnostic evaluations
Psychological or neuropsychological testing
Current medication lists
Risk assessments
Collateral information from current treatment providers

Clinical information may be submitted securely by fax or encrypted email.

The Director of Admissions may also collaborate with the individual’s current treatment team to gain a more complete understanding of clinical presentation, strengths, support needs, and treatment goals.

Following review, Wild Acre determines whether the program is an appropriate fit and recommends the level of support that best matches the individual’s needs. If admission is appropriate, the Director of Admissions works with the individual, family, and treatment team to coordinate a smooth transition into the program.

A continuum of structured, community-based support.

Wild Acre provides flexible, person-centered clinical support across multiple levels of care, including DMH Licensed Residential Programs, transitional group homes, supportive apartments, case management, and home care services.

The continuum is designed to support adults at different stages of stability, functioning, and readiness for independence.

Some individuals may require the consistency of long-term residential support.

Others may benefit from a transitional residence focused on stabilization, skill development, and next-step planning.

Supportive apartments and home care services may be appropriate for individuals who are ready for a greater level of autonomy with continued case management, coaching, and clinical coordination.

Across levels of care, Wild Acre emphasize:

Individualized treatment planning
Collaborative care
Therapeutic milieu
Community integration
Medication support
Daily living skill development
Continuity with outpatient therapeutic and psychiatric providers

Clinical and functional fit considerations.

Wild Acre supports adults ages 18 and older experiencing a range of psychiatric, neurodevelopmental, and co-occurring presentations.

Individuals may present with thought disorders, including schizophrenia or schizoaffective disorder, bipolar disorder, anxiety disorders, depressive disorders, obsessive-compulsive disorder, personality disorders, autism spectrum disorder, co-occurring substance use concerns, history of substance use, or other chronic mental health challenges.

Fit is evaluated through both diagnostic and functional lenses.

Potential referral considerations may include:

Need for residential, transitional, apartment-based, or in-home support
Ability to participate safely in a community-based therapeutic environment
Need for clinical case management and treatment coordination
Daily living skill, executive functioning, or routine-management needs
Medication support needs and progress toward medication independence
Social skill development, interpersonal effectiveness, and relationship support
Vocational, educational, volunteer, or other meaningful outside-structure goals
Transition from hospitalization, residential treatment, family home, or another structured setting
Established or emerging outpatient treatment relationships
Family involvement and support system considerations
Substance use history, current sobriety, and recovery support needs when relevant

Admission decisions are based on a comprehensive review of the individual’s current functioning, treatment history, support needs, goals, stability, and readiness for the recommended level of care.

Continuity of care is central to the model.

Wild Acre is designed to work in close collaboration with an individual’s existing outpatient treatment team whenever possible.

This may include psychiatrists, therapists, case managers, hospitals, educational or vocational supports, family members when appropriate, and other providers involved in the individual’s care.

Our clinical case management model supports coordination around treatment goals, medication routines, appointment follow-through, crisis intervention and de-escalation, family communication when appropriate, and transition planning.

Wild Acre provides clinical case management, behavioral support, daily structure, and real-time coaching. Individual psychotherapy and psychiatric medication management remain outpatient-based.

This distinction is intentional. Maintaining outpatient therapeutic and psychiatric relationships supports long-term continuity of care and helps residents establish sustainable roots in the community as they transition to lower levels of support and greater independence.

Support within daily life and community context.

Wild Acre’s services vary by program and level of care, but may include:

Clinical case management with trained mental health clinicians
Treatment planning and goal setting
Daily or regular meetings with designated case managers, depending on level of care
24-hour on-site staff support in group home settings
Medication administration, support, and training toward medication independence
Communication with prescribers and outpatient providers
Psychiatric consultation or guidance to staff, as appropriate by program
Crisis intervention, de-escalation, and conflict resolution support
Schedule and calendar management
Daily living and executive functioning support
Coping skills development and coaching
Health, wellness, art, and expressive groups
Vocational assessment, educational planning, and outside-structure support
Transportation support for clinical and medical appointments
Support learning to navigate the community independently
Family communication and support meetings when appropriate
Transition and aftercare planning

The model is intentionally relational and community-based. Support is woven throughout formal meetings, daily routines, interpersonal interactions, shared living environments, and real-world situations where residents are practicing and generalizing skills.

Important scope of care.

Wild Acre is a community-based residential mental health program for adults who are able to participate safely in a structured, supportive living environment.

Wild Acre is not an acute inpatient psychiatric hospital, locked treatment facility, detoxification program, or crisis stabilization program.

Our programs provide clinical case management, behavioral support, skill development, daily structure, residential support, and ongoing care coordination. Residents continue to receive individual psychotherapy, psychiatric medication management, and other specialized clinical services from their established outpatient providers while enrolled in our programs.

Wild Acre may not be the appropriate setting for individuals who require:

Inpatient psychiatric hospitalization
Medical hospitalization
Medical detoxification
Continuous one-to-one supervision
A locked treatment environment
A higher level of safety monitoring than can be provided in a community-based setting

During the admissions process, our team carefully reviews each referral to determine whether Wild Acre is the most appropriate level of care. If another setting would better meet an individual’s needs, we will communicate that recommendation and, whenever possible, help guide the referral toward a more appropriate level of care.

Wild Acre is a private-pay program and does not accept insurance. Program fees and payment expectations are reviewed during the admissions process.
